I start with a profiling pass to find where the time actually goes: slow database queries, N+1 patterns, inefficient Eloquent usage, missing indexes, oversized result sets, unnecessary memory allocation, badly configured caching and queues. Then I prioritize by impact, fix the biggest wins first and measure the before/after so improvements are visible, not theoretical.
